Consultation
Fees
The consultation
fee is $85. This fee is paid at the time of the
consultation,
but if the surgery is performed, it is deducted from
the surgical
fee. If the surgery is not done, the consultation
fee is
not refunded.
Payment
of Surgical Fees
The surgical
fees must be paid ten days prior to the surgery date
Cancellations
Your surgical
fee will be refunded, less the consultation fee of
$85, if
the cancellation notice is received at least three working
days prior
to your surgery date. If the cancellation is received
with less
than three working days notice, there will be a $300
cancellation
fee and the $85 consultation fee will be withheld
from your
refund.
Touchups
Occasionally
a touch-up procedure is necessary and those will be
addressed
on an individual basis.
